Does removing the concrete slab cost extra?

It can. Breaking up and hauling away a concrete pad is heavier, more involved work than taking down the frame and roof, so a job that includes slab removal costs more than one that leaves the pad in place. Tell us whether you want the slab gone and we will include it in the quote.

Is it cheaper to clear out the garage than to demolish it?

They are two different jobs. Clearing out means removing everything inside and leaving the building standing; demolition means taking the structure down as well. If you only need the contents gone, a cleanout costs less than a full teardown, tell us which you need and we will quote it.
